Join the Practice

Registration

New patients who live within the Practice boundaries are welcome, you must first contact reception to check if your address falls within the practice boundary.

To register, please bring your medical card to reception. If you do not have a medical card an HS200 form will be given to you for completion, providing you have continuously lived within Northern Ireland.  All other patients will be given a form HSC/R1 to complete. New patients registering with the Practice are required to produce current: photographic ID, passport, proof of address, work permit or visa (if applicable) and their 10 digit Health and Care Number.  

If you have had confirmation that you can register with the surgery you may print your own forms to complete at home by using the following:-

If your records are with a GP surgery within the UK use an HS 200 Form HS200 to register when your records are held with a UK GP Surgery

If you do not currently have a GP in the UK then you must use an SCR1 form HSCR1 for those registering from outside of the UK

You may be asked to attend for a new patient medical consultation.

Where patients are requesting to join the Practice list, the Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive immediate and necessary treatment from the local GP practice. After 14 days you will need to register as a permanent patient or return to your own GP.

To register as a temporary patient simply contact the surgery.
